In Malta, Most never MORE than four in every five suicides in Malta are men. Most of them never spoke to anyone about their mental health strug- gles—not a doctor, not a thera- pist, not a friend. And those men who do ask for help as a result of mental health issues tend to arrive late. Between January 2015 and June 2021, 131 of the 162 sui- cides registered in Malta were men. Research shows that only 19.2% of those who died had ever been in contact with men- tal health services. Of the 1,324 clients using the main therapy services of the Richmond Foun- dation, a mental health support community, in 2025, 791 were women, and 467 were men. The disparity is glaring. But what keeps men away from seeking help? Matthew Paris was 15 when he could no longer manage his episodes of psychosis quietly. For two years before that, he had managed just about. With- drawing from friends, slipping at school, saying nothing to an- yone. When he finally spoke, it was not because he was ready. "What ultimately made me speak out was, in inverted com- mas, a fear of getting caught almost. It was more the fear of getting in trouble for having psychosis and not having said anything so far," he says. "It got to a point where I couldn't keep it to myself any longer. It was either speak out, or something even scarier than that could happen—something I could not understand." He is now a learning and de- velopment officer and patient advocate at the Richmond Foundation. He works with men who are still in the place he was, deciding whether to say something or how long they can hold out. He knows what that calculation feels like from the inside. Paris says men rarely seek help on their own initiative. When they do come, it is usually be- cause someone in their life, a partner, a friend, a brother, pushed them toward it.
